You make a Fortitude save against the DC of the spell or effect that bestowed the negative level, and if you succeed, the negative level disappears. If you fail, the negative level remains (though you still have another chance to remove the negative level after 24 hours). You can only make one attempt to shrug off a negative level, and can only attempt to shrug off one negative level per round.
